During your initial visit to our clinic, we will thoroughly review your medical history, conduct a pelvic exam, and collect a urine sample for analysis. Dr. Socas will personally meet with you to discuss her findings, offer treatment recommendations, and arrange further testing if needed, such as Urodynamics and/or Cystoscopy.
As a new patient, we kindly request that you arrive 20-30 minutes prior to your scheduled appointment time. This allows ample time for completing the registration process and any necessary paperwork. For your convenience, you may download and fill out the forms provided below before your appointment. Alternatively, if you have received a user id and password, you may also submit these forms through our Secure Patient Portal. These forms are crucial for Dr. Socas to better understand your condition and tailor a treatment plan to address your specific concerns. Your cooperation in completing them accurately is greatly appreciated.
